Post-operative team care is an important service we provide to optimise patient care.
The APAC service in Northern Sydney is an affiliated team of specially trained nurses, physiotherapists and social workers that faciliate early discharge from hospital.
The APAC service is a "Hospital in the Home" service that provides intense, short term, interdisciplinary acute health care and management to patient's as direct substitution for in-patient hospital care.
Some of their services include home visits, wound care, drain management and liaison with other medical and general practitioners. This service enables timely discharge from hospital and ongoing postoperative care and support.
